All applicants are required to forward the following information for evaluation:
- Confirmation of completion of graduate studies educational status (degrees)
- The applicant must be directly affiliated with an existing FASD Diagnostic Clinic
and/or Diagnostic Physician.
- Affirmation of current good standing with respective mandatory professional association
- Documentation of accredited hours in specified area of clinical practice to date and summary of skills
- Two letters of reference (include telephone numbers / e-mail address)
Some applicants may not be accepted based on the review of submissions.
Certificate of Completion and Clinical Registry Status (pending all course requirements achieved). |
